This breach caused a small internal implosion, which in turn showered the gas burner with debris and soot.

In technical terms, a hole in the heat exchanger is a "safety-critical" fault. It means the unit can no longer safely contain the combustion process, potentially allowing carbon monoxide or flames to escape. For the safety of the household, Nick had no choice but to condemn the boiler immediately.
